To make things easier, we have made available many of our usual forms in an online format. Please see below for the form you are after. It is always advisable to contact the school office to let us know you have submitted a form for our attention. Click on the titles below to follow each form link.
End of Day - Collection Routines
This form is for parents to inform the school where they would like us to release children at the end of the day rather than waiting to hand over pupils (to an adult) in the playground. PLEASE NOTE: this form is not needed for pupils in Year 5 and 6 who are dismissed independently from the classroom each day.
Please use this form to report any pupil absences. Please note that reported absences may be followed up with a call from school.
Request to Administer Medication
If your child is unwell, they should stay at home. If your child is well but has been prescribed medicine by your GP. Please ensure you complete this form if you require school staff to administer this medicine in school. All medicine should be sent in to school in the original packaging, with instructions and means of administration i.e. medicine spoon.
Before & after school club (The Hub) - waiting list
Our before and after school clubs are currently full. If you would like to be added to our 'Hub' waiting list, please complete the linked form above.
Application for leave of absence from school
If you are looking to book time off school during term time, please complete this form. It would also be worth reading our attendance policy.

Covid-19 Home / School Agreement
To ensure all our parents have read and understand the changes we have put in place to protect the safety of our pupils and school community, we are asking parents to complete a specific home/school agreement before your child returns to school.
